چکیده

A 50-year-old white man was admitted to the hospital for evaluation of abnormal findings on chest film. Three weeks prior to admission, he developed left pleuritic chest pain associated with some mild cough. No fever was noted at that time. Radiologic evaluation disclosed multiple, bilateral pulmonary nodules, some of which were suspected to be cavitary in nature. The patient had a seven-year history of regional ileitis, and a right hemicolectomy and distal ileectomy had been performed. His medications upon admission to the hospital were prednisone, 17.5 mg every other day, salicylazosulfapyridine (Azul, fidine) 2 gm daily. The patient was a nonsmoker and nondrinker. He had no history of previous pulmonary disease. He operated a fruit farm and actively engaged in work duties, which included tilling soil, picking fruits, and spraying trees. He denied any weight loss, shortness of breath, hemoptysis, pedal edema, paroxysmal nocturnal dyspnea, or orthopnea. Physical examination revealed the patient to be in no acute distress. Except for a surgical scar in his abdomen, physical examination was normal. Chest x-ray films obtained at admission and full chest

A case of pulmonary cryptococcosis caused by capsule-deficient cryptococcus neoformans in an immunocompetent patient.

Cryptococcus neoformans is a common cause of opportunistic infection in patients with human immunodeficiency virus (HIV) and others with cell-mediated immunodeficiency, such as patients with solid-organ-transplant-related immunosuppression, patients receiving corticosteroids or other immunosuppressive therapy, and patients with lymphoid malignancy.1,2 The central nervous system (CNS) is the mos...

Clinical and radiologic manifestations of pulmonary cryptococcosis in immunocompetent patients and their outcomes after treatment.

PURPOSE We aimed to investigate clinical and radiologic manifestations of pulmonary cryptococcosis in immunocompetent patients and their outcomes after treatment. MATERIALS AND METHODS We retrospectively reviewed the medical records, initial and follow-up chest computed tomography scans and/or radiographs for initial clinical and radiologic manifestations and outcomes following antifungal tre...

Pulmonary cryptococcal infection presenting with multiple lung nodules

Pulmonary infections from the environmental fungus Cryptococcus gattii (C. gattii) are notable for cryptococcomas, which are usually solitary and can be very large. As with infections with Cryptococcus neoformans (C. neoformans) patients can have concomitant cryptococcal meningitis; however, unlike for C. neoformans, infections with C. gattii often occur in immunocompetent patients. The highest...

Case of Pulmonary Cryptococcosis Mimicking Hematogeneous Metastases in an Immuocompetent Patient: Value of Absent 18F-Fluorodeoxylucose Uptake on Positron Emission Tomography/CT Scan

The radiologic appearance of multiple discrete pulmonary nodules in immunocompetent patients, with cryptococcal infection, has been rarely described. We describe a case of pulmonary cryptococcosis, presenting with bilaterally and randomly distributed nodules on a computed tomography, mimicking hematogeneous metastases. Positron emission tomography does not demonstrate 18F-fluorodeoxyglucose (FD...

PR3-ANCA-positive Pulmonary Cryptococcosis

A 68-year-old man had multiple left pulmonary nodules with serum positivity for proteinase 3 anti-neutrophil cytoplasmic autoantibody (PR3-ANCA) and negativity for cryptococcal antigen (Picture 1). We initially regarded him as having granulomatosis with polyangiitis (GPA). He subsequently underwent a video-assisted surgical lung biopsy for the diagnosis (Picture 1, arrowheads). Hematoxylin and ...
